The Comprehensive Stroke Program at Palmetto General Hospital has been rated as one of the best in the Nation over the course of time, and the medical institution wanted to recognize Medical Director Dr. Ritesh Kaushal, and his team in an exceptional event for the years of service and contribution to patient´s recovery and beyond.
Established in 2013, Palmetto General Hospital´s Comprehensive Stroke Program has been of crucial importance for those who suffer a stroke in the Hialeah community, as well as the entire Miami-Dade and surrounding areas.
Let´s remember first what a stroke is. It’s a medical condition in which poor blood flow to a part of the brain causes cell death, and this situation may cause signs and symptoms that include an inability to move or feel on one side of the body, problems understanding or speaking, dizziness, or loss of vision to one side.
Also, let’s recall, a Comprehensive Stroke Program refers to the highest level of stroke care, meaning it is equipped to handle the most complex stroke cases, including various types of hemorrhagic and ischemic strokes, with a team of specialized experts like vascular neurologists, neurosurgeons, and advanced imaging capabilities available 24 hours a day.
“A stroke is one of the most debilitating disorders,” Dr. Kaushal pointed out.
“It not only affects the patient himself, but it also ends up affecting the whole family. If someone is paralyzed, completely on one side, bedridden, family members (or friends) need to take care of the patient,” he stated.
In other words, family members or friends, and the patient, must learn how to deal with this medical condition.
“I’ve had so many patients and their family members who have to end up leaving their job. So, it not only just affects the patient. It also affects the family. It affects the community, and overall, eventually the country as well,” Dr. Kaushal commented.
First 60 minutes
The first hour in stroke care is called golden hour. “And within that much time, the more you can do in terms of management, any kind of intervention, the better the outcomes of the patient,” Dr. Kaushal emphasized.
Basically, a clot goes up, locks a blood vessel in the brain, and that whole part of the brain now is no longer getting blood. “The clock starts at that time. Usually, the patients themselves cannot notice the situation, so someone else, maybe a family member, has to recognize that and call 911 immediately to bring the patient to the most appropriate facility.”, he highlighted.
Teamwork
This is teamwork when a group of professionals work together to achieve a common goal. “The rescue, the emergency room, the coordination with the Comprehensive Stroke Program, the neurointensivist, Dr. Kaushal. And then, the rehabilitation, speech pathology, therapy. It’s really a team approach to the care of these patients,” Palmetto General Hospital´s Chief Executive Officer Dr. Anais Cortes, affirmed.
“We have one of the best outcomes in terms of complications, mortality rates nationwide. It’s all about the team, obviously led by Dr. Kaushal,” she added.
“And that’s the reason we’re so successful. I am extremely, extremely proud of this program. It´s nationally recognized and has really placed Palmetto General Hospital at a different level,” Dr. Cortes pointed out.
“Every single day we have patients from all ages. The 30s, 40s, 50s, 60s, 70s, 80s. We really do miracles daily here,” she underlined.
Experiences
Everyday Palmetto General Hospital treats patients suffering from strokes, and Captain Ivanov, from the City of Hialeah Fire Department Rescue Division, was on duty when he got a call about someone having a medical situation.
“We went to the patient. We basically did an exam, and we quickly realized that he may be having a stroke,” Captain Ivanov recalled.
Meanwhile, during the examination, the patient’s son was on the phone. “He was adamant. He was categorically not taking his dad to the hospital, but I tried to convince him. I told him everything. I was very, very convinced that Palmetto General Hospital was the right place for this patient. Not only because I know the doctor, but because this is the only comprehensive facility in Hialeah that can take care of that. Knowing what I know, Dr. Kaushal started this program almost 12 years ago, and I was there from basically day one,” Captain Ivanov affirmed.
While the patient was being transported to Palmetto General Hospital, Captain Ivanov called Dr. Kaushal, just like it’s part of the protocol: “I let him know we were bringing a patient. He was busy performing a medical procedure, but he was nice enough to receive my call and tell me that he would be seeing the patient.”
A few hours later, it was already like 12 o’clock at night, time to go to bed, but Captain Ivanov was happy when he got the text message from Dr. Kaushal explaining what had happened.
“The patient had a full left carotid occlusion [One of the main arteries that supply blood to the brain], and his full left side of the brain was not getting blood supply. Which means that within 20-30 minutes or however long, the patient is either going to be dead or will have deficit for the rest of his life”, Captain Ivanov explained.
And this was an extreme situation. The patient was taken care of. He was treated on time, thanks to the quick response of the whole team.
In the following days, the son reached out to Captain Ivanov that he wanted to meet Dr. Kaushal and thank him: “They met, and the patient’s son thanked the doctor and his team, and us, for saving his father. And I was very thankful for that, as well as the whole team.”
Today, after completing a long treatment, and rehab, and working with family, the patient is recovering.
“This proves what kind of service Palmetto General Hospital provides to the community,” Captain Ivanov highlighted.
“The whole team, the rescue, the emergency room, the Comprehensive Stroke, the hospital, they were amazing,” he exclaimed.
Prevention
To prevent a stroke, Dr. Kaushal basically advises you must focus on maintaining a healthy lifestyle, which means controlling blood pressure, managing cholesterol levels, exercising regularly, eating a balanced diet, maintaining a healthy weight, limiting alcohol intake, getting regular medical checkups to monitor your health conditions, and, if you smoke, quit smoking.

El Programa Integral de Accidentes Cerebrovasculares del Palmetto General Hospital ha sido calificado como uno de los mejores del país a lo largo del tiempo, y la institución médica quiso reconocer al director médico Dr. Ritesh Kaushal, y su equipo en un evento excepcional por los años de servicio y contribución a la recuperación del paciente y más.
Establecido en 2013, el Programa Integral de Accidentes Cerebrovasculares del Palmetto General Hospital ha sido de crucial importancia para quienes sufren un derrame cerebral en la comunidad de Hialeah, así como en todo Miami-Dade y sus alrededores.
Recordemos primero que el derrame cerebral es una afección médica en la que el flujo sanguíneo deficiente a una parte del cerebro provoca la muerte celular, y esta situación puede causar signos y síntomas como incapacidad para moverse o sentir un lado del cuerpo, problemas para comprender o hablar, mareos o pérdida de la visión en un ojo.
Además, un Programa Integral de Accidentes Cerebrovasculares implica el más alto nivel de atención, lo que significa que el centro médico está equipado para manejar los casos de accidentes cerebrovasculares más complejos, incluidos varios tipos de hemorrágicos e isquémicos, con un equipo de expertos especializados como neurólogos vasculares, neurocirujanos y técnicas avanzadas de imágenes disponibles las 24 horas del día.
«Un derrame cerebral es uno de los trastornos más debilitantes», señaló el Dr. Kaushal.
“No sólo afecta al paciente, sino que acaba afectando a toda la familia. Si alguien está paralizado, completamente de un lado, postrado en cama, los familiares (o amigos) deben cuidar del paciente”, afirmó.
En otras palabras, los familiares o amigos, y el paciente, deben aprender a bregar con esta afección.
“He tenido tantos pacientes y sus familiares que terminan dejando su trabajo. Por lo tanto, no sólo afecta al paciente. También afecta a la familia. Afecta a la comunidad y, en general, también al país”, comentó el Dr. Kaushal.
Primeros 60 minutos
La primera hora de atención al ictus se llama hora dorada. “Y dentro de ese tiempo, cuanto más se pueda hacer en términos de manejo, cualquier tipo de intervención, mejores serán los resultados para el paciente”, enfatizó el Dr. Kaushal.
Básicamente, un coágulo bloquea un vaso sanguíneo en el cerebro y toda esa parte del cerebro deja de recibir sangre. “El reloj empieza a contar el tiempo en se momento. Generalmente, los pacientes no notan la situación, por lo que alguien, tal vez un familiar, tiene que darse cuenta y llamar al 911 de inmediato para llevar al paciente al centro médico más adecuado”, destacó.
Trabajo en equipo
Se trata de un trabajo en equipo cuando un grupo de profesionales desempeñan sus funciones coordinadamente para lograr un objetivo común. “El rescate, la sala de emergencias, la coordinación con el Programa Integral de Ictus, el neurointensivista, el Dr. Kaushal. Y luego, la rehabilitación, la patología del habla, la terapia. Es realmente un enfoque de equipo para atender estos pacientes”, afirmó la directora ejecutiva de Palmetto General Hospital, Dra. Anais Cortés.
“Tenemos uno de los mejores resultados en términos de complicaciones y tasas de mortalidad a nivel nacional. Se trata del equipo, obviamente dirigido por el Dr. Kaushal”, añadió.
“Y esa es la razón por la que tenemos tanto éxito. Estoy extremadamente, extremadamente orgullosa de este programa. Es reconocido a nivel nacional y ha colocado al Hospital General Palmetto en un nivel diferente”, señaló la Dra. Cortés.
“Todos los días tenemos pacientes de todas las edades. De 30, 40, 50, 60, 70, 80. Aquí realmente hacemos milagros a diario”, subrayó.
Experiencias
Todos los días Palmetto General Hospital trata a pacientes que sufren accidentes cerebrovasculares, y el capitán Ivanov, de la División de Rescate del Departamento de Bomberos de la ciudad de Hialeah, estaba de servicio cuando recibió una llamada sobre alguien que sufría una situación médica.
“Acudimos al paciente. Básicamente le hicimos un examen médico y rápidamente nos dimos cuenta de que podría estar sufriendo un derrame cerebral”, recordó el capitán Ivanov.
Mientras tanto, durante el examen, el hijo del paciente llamó por teléfono. “Fue resistente. No quería que lleváramos a su papá al hospital, pero traté de convencerlo. Le expliqué. Yo estaba muy, muy convencido de que Palmetto General Hospital era el lugar adecuado para el paciente. No sólo porque conozco al médico, sino porque este es el único centro integral en Hialeah que puede encargarse de eso. Sabiendo lo que sé, el Dr. Kaushal comenzó este programa hace casi 12 años y yo estuve allí básicamente desde el primer día”, afirmó el Capitán Ivanov.
Mientras transportaban el paciente al Hospital General Palmetto, el capitán Ivanov llamó al Dr. Kaushal, como parte del protocolo: “Le informé que traíamos un paciente. Estaba ocupado realizando un procedimiento médico, pero tuvo la amabilidad de recibir mi llamada y decirme que vería al paciente”.
Unas horas más tarde, ya eran como las 12 de la noche, hora de irse a la cama, pero el Capitán Ivanov se alegró cuando recibió el mensaje de texto del Dr. Kaushal explicando lo sucedido.
“El paciente tenía una oclusión completa de la carótida izquierda [una de las principales arterias que suministra sangre al cerebro] y todo el lado izquierdo del cerebro no recibía suministro de sangre. Esto significa que en 20-30 minutos o el tiempo que sea el paciente moriría o tendría déficit para el resto de su vida”, explicó el capitán Ivanov.
Era una situación extrema. El paciente fue atendido. Fue atendido a tiempo, gracias a la rápida respuesta de todo el equipo.
En los días siguientes, el hijo del paciente expresó al capitán Ivanov su deseo de conocer al Dr. Kaushal y agradecerle: “Se reunieron y agradeció al médico y a su equipo, y a nosotros, por salvar a su padre. Y estuve muy agradecido por eso, al igual que todo el equipo”.
Hoy, después de completar un largo tratamiento, rehabilitación y trabajo con familiares, el paciente se recupera.
“Esto demuestra el tipo de servicio que el Hospital General Palmetto ofrece a la comunidad”, destacó el capitán Ivanov.
“Todo el equipo, el rescate, urgencias, el personal del programa, el hospital, se portaron de una manera increíble”, exclamó.
Prevención
Para prevenir un derrame cerebral, el Dr. Kaushal básicamente aconseja concentrarse en mantener un estilo de vida saludable, lo que significa controlar la presión arterial, controlar los niveles de colesterol, hacer ejercicio regularmente, llevar una dieta equilibrada, mantener un peso saludable, limitar el consumo de alcohol, hacerse chequeos médicos periódicos para controlar la salud y, si fuma, dejar de fumar.
