We offer comprehensive medical, social, and recreational services to adults over the age of 55 who are living with a chronic illness or disability and need coordinated medical care to continue living as independently as possible at home or in the community.
PACE is truly all-inclusive care. The PACE program provides seniors with primary medical care, specialty doctors, medical imaging, restorative therapies, rehabilitation, nutritional counseling, home delivered meals, social work, home care services, prescription drugs, medical equipment and supplies, mental health services, transportation, Adult Day care and much more. Mount Sinai Eldercare’s services are offered in our PACE center, at home, or in the community.
Eldercare is conveniently located at 6050 West 20th Avenue in Hialeah. It was designed to offer you (or a loved one) all the services you need in one convenient location, from medical care, therapy (physical, occupational, and speech) services, dietary assistance, and social services. Meet new friends, participate in stimulating activities, and enjoy delicious and healthy meals. Conceptualized and designed to engage and inspire participants, Mount Sinai Eldercare offers unmatched care, support, and compassion for those who qualify.
The Mount Sinai Eldercare Center is open Monday through Friday, 8 a.m. to 5 p.m. We are pleased to be able to offer our community access to a new facility and expert care team.
